This role entails conducting assessments and working with 15-30 Veteran households on a plan for housing and managing the individual cases (non-clinical). Title: Case Manager Location: Vineland, NJ 08360 (primary office) Duration: 4 month contract-to-hire Pay Rate: $24/hr Schedule: Monday-Friday, 8:30-4:30 PM, 30 minute lunch. Travel Requirement: 15-30% in Southern NJ counties, vehicle provided Overview: The Case Managers provide Case Management for approximately 15-30 veteran households at a time, supporting development and achievement of individual housing stability plans. Consulting with potential clients to assess needs and determine program eligibility Working with clients to create individualized housing stability plans with client-specific goals Monitoring clients from the time of intake until they exit the program Ensuring that clients obtain needed supportive services Key processes include: Providing ongoing support and expertise through assessment, planning, implementation, and overall evaluation of the veteran's needs as drawn from his/her goal/action plans. Obtaining information contributing to the client's situation and evaluating these, along with the client's capabilities/skills. Collaborating weekly with the Housing Coordinator, Social Worker and Income Stabilization Specialist on action/service plans, coordinating approaches for successful outcomes. Following up to determine quantity and quality of service provided to the veterans and the status of his/her care 70% of the Case Manager's work effort should be to case management and approximately 30% with outreach. Responsibilities: Conduct outreach to identify and provide services for target populations Develop effective plans for clients, in coordination with the Social Worker Provide Case Management with a focus on housing first Connect clients to VA resources/benefits Connect clients to mainstream resources/benefits Support and execute fundraising events Client Satisfaction: Assist clients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner. Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated. Identify yourself in a pleasant and positive manner. Participates in case management meetings. Coordinate, through case management, an active behavioral health treatment plan when appropriate. Provides educational presentations related to functional life skills, time management, anger management, communication skills, values clarification, budgeting, independent living and parenting skills. Requirements: Software experience including but not limited to Microsoft Office Suite. The ability to communicate with all levels of the organization. This position also requires a solid, self-directed individual who can excel both independently and as a team player in a fast-paced environment. Candidates must be able to complete tasks at accepted levels of quality. Identifies and resolves problems in a timely manner; gathers and skillfully analyzes information Speaks clearly and convincingly Decision-making; staff in planning; available to staff; provides regular performance feedback; and develops subordinates' skills and encourages development.
