This position is a behavioral health consultant role in the primary care clinic that works to pro-actively support a smooth interface between primary care and specialty behavioral health services. The employee provides recommendations regarding behavioral health interventions, lifestyle wellness coaching along with facilitation of referrals, brief interventions and follow up.
- Responsible for working with the Primary Care Provider to recognize and address psychological symptoms and conditions using the bio-psychosocial model. Acts as the behavioral health consultant of the primary care team. Performs screening, assessment, treatment plans, and brief treatment for a wide range of psychosocial disorders using brief therapy. Works with providers to monitor and manage “at risk” and “chronic” patients.
- Supports provider decision making and builds on provider-patient interventions. Provides effective triage of patients in need of specialty lifestyle coaching and/or behavioral health services that are provided at the in Specialty Behavioral Health Clinic as well as outside organizations.
- Coordinates and works with other team members to ensure the efficient and effective flow of patients through the primary care and behavioral health departments.
- Establishes and maintains liaison with other related professional services to ensure that clinical requirements are provided for patients. Develops and maintains a trusting and partnering relationship with assigned patients in order to assist in the success of treatment.
- Obtains complete and accurate information in a courteous, respectful and culturally sensitive manner. Attends medical staff meetings, training sessions, and planning and problem solving sessions. Participates in regularly scheduled individual and group supervisory sessions.
- Disseminates information to primary care team on recent advances in the field.
- Maintains all required behavioral health documentation including screening tools, assessments, treatment plans, progress notes, and discharges. Patient documentation is documented clearly and timely in order to provide adequate information for other health care providers.
- Must meet a minimum expectation of 25 billable client encounters per week.
- Master’s Degree in Social Work, Psychology, Counseling or related discipline from an accredited educational institution.
- Employees should have a thorough understanding of the cultures and the needs of SEARHC beneficiaries, and of the general culture and needs of Alaska Natives and American Indians. Such knowledge is critical in order to ensure the achievement of the mission and vision of SEARHC: working together with the Native community to provide the best services with Native staff who value the family and family wellness as the heart of the community.
- 2 years of supervised clinical work experience.
- Trained in behavioral medicine and health psychology concepts preferred.
- Trained in social systems and family aspects of psychological and medical conditions preferred.
- Must be licensed as a: Licensed Clinical Social Worker (LCSW).
Knowledge, Skills & Abilities
- Assessment techniques, diagnosis and treatment of psychiatric and substance use disorders in children, adolescents and adults.
- Basic nutrition, physical activity and tobacco cessation recommendations for physical health and disease prevention
- Sexual abuse, domestic violence, and substance abuse issues; as well as evaluating suicidal individuals or individuals in crisis and managing their safe disposition.
- A broad repertoire of psychotherapeutic skills from a variety of theoretical perspectives (e.g.: motivational interviewing, cognitive behavioral therapy and family therapy).
- Exposure to and training in evidence based procedures for complicated conditions, such as severe depression, obsessive compulsive disorder, diabetes, tobacco addiction.
- Communication skills sufficient to report patient activities and deviations from normal; ability to interact positively and effectively with patients and families.
- A high level of skill in empathic interpersonal relationships is a priority for this position since the perception of the patient must be that the staff is interested in attending to their needs.
- Skill at interpersonal relationships to work as an effective team member within a department which is focused on team building.
- Maintain strict confidentiality in regards to patient care information and chart contents as use of patient record and privacy of individuals must be protected to the fullest.
- Maintain documentation in compliance with State, Federal, Professional and Ethical guidelines.
- Consistently utilize professional communication and conflict resolution skills with patients and staff.
- Efficiently and effectively manage multiple clinical tasks and be self-directed.
- Accurately and routinely utilize the DSM V and comprehensive clinical assessment information to accurately diagnose individuals treated.
- Use the computer system in order to obtain and document patient information.
- Utilize different therapeutic modalities including: group therapy, individual, child, family, couples counseling, family support services and wrap around services.
- Utilize assessment information and collateral information to conceptualize patient psychopathology.
- Effectively coordinate with multi-disciplinary team and SEARHC providers in accomplishing goals/objectives of treatment plan.
