Job Description

As a key member of the team, the Case Manager oversees all aspects of the clinical experience of our patients with neurological impairment (e.g., traumatic brain injury, stroke, anoxia, etc.).

Acting as the advocate for our patients, our Case Manager prepares outcome-oriented Care Plans through a collaborative process with the patient, their family, the payor, and the medical and clinical team members. The Case Manager works with our clinical team to ensure that services provided are effective in achieving optimal clinical outcomes. Ultimately, the Case Manager ensures that quality expectations are achieved. At Learning Services, a core value is customer satisfaction, and the Case Manager works to achieve satisfaction through an alignment of expectations with all parties involved.

Position Requirements:

  • Demonstrates effective verbal and written communication skills
  • Demonstrates ability to work independently, take initiative, and consistently exercise sound discretion and judgment
  • Experience in disability management and/or clinical experience in a neurorehabilitation-related field
  • Advanced degree in Clinical and/or a healthcare related field (ex. MSW, LCSW, Rehab Counselor)
  • For management & coverage of the Program, willingness to be part of telephonic on-call rotation.
  • Ability to lift (50) pounds unassisted
  • Preference for CCM certification (Commission for Case Manager Certification)
