As a key member of the team, the Case Manager oversees all aspects of the clinical experience of our patients with neurological impairment (e.g., traumatic brain injury, stroke, anoxia, etc.).
Acting as the advocate for our patients, our Case Manager prepares outcome-oriented Care Plans through a collaborative process with the patient, their family, the payor, and the medical and clinical team members. The Case Manager works with our clinical team to ensure that services provided are effective in achieving optimal clinical outcomes. Ultimately, the Case Manager ensures that quality expectations are achieved.
