Assist with the coordination of all components of the Nursing Home Transition and Diversion (NHTD) Waiver on Long Island in conjunction with the Department of Health. Conduct telephone referrals and intakes on potential participants. Make referrals to available resources in the community if an individual is determined not appropriate for the waiver. Determine waiver participant eligibility. Complete enrollment procedures according to program guidelines, including explanation of Waiver philosophy and participant choice. Refer medically complex cases to the Nurse Evaluator. Provide prospective participants with a choice among available waiver Service Coordinators. Review referrals and intakes of support staff if not the RRDC staff completing the aforementioned evaluations. Review, monitor and approve waiver participant Service Plans developed by the participant and the Service Coordinator. Support outreach and public education efforts of Regional Resource Development Center (RRDC). Conduct at least 8 provider meetings a year in accordance with Work Plan. Assist with the coordination of all components of the respective waiver program for the Long Island region in conjunction with the Department of Health. Make referrals to the appropriate LDSS to determine Medicaid eligibility; if appropriate, offer applicant the opportunity to participate in the respective Waiver. Work in coordination with the LDSS to appropriately use Medicaid State Plan and waiver services for eligible applicants. Work closely with DOH to disseminate public information and education regarding the needs of people who may meet eligibility criteria (and their families) of the respective waiver. Develop strong working relationships with regionally based inpatient rehabilitation units and long-term care facilities to identify prospective participants and facilitate community re-entry from these institutions. Provide data as needed as part of the quarterly reports and at other times upon request. As needed, manage Provider Enrollment and Training by interviewing potential waiver service providers and making enrollment recommendations to DOH based on factors such as the provider’s understanding of the waiver’s philosophy of participant choice and control, and the assessed ability to deliver high quality, cost effective services. Train service providers and offer technical assistance on the needs of the waiver participants, development and implementation of comprehensive waiver Service Plans, and standard documentation and reporting requirements. Conduct trainings for providers, including but not limited to DOH approved curriculum. Maintain participant applications, Service Plans, and reports in a manner consistent with State standards for electronic transfer and information sharing. Attend team meetings as necessary. Make recommendations, based on experience with waiver activities and ongoing administration of the program, to supervisor and/or DOH for improvements to waiver policies and procedures. Attend inter and intra agency meetings as required.
