Job Title: Substance Abuse Social Worker Company: Prevention Counseling Services Location: Glasgow, KY Description: Job Purpose: Supports clients by providing outpatient drug, alcohol, or other substance abuse counseling; individual and family therapy Duties:  Conducts a detailed assessment of each client and develops (in collaboration with the client) an individualized treatment plan which addresses the identified needs.        Provides a minimum average daily productivity of five (5) billable hours per work day, unless otherwise specified in writing.  Documents all treatment and case management activity in accordance with State, Medicaid and PCS guidelines and generally accepted standards.  Completes all paperwork and assignments in a timely fashion. (Clinical paperwork is to be completed no later than 48 hours (work days) from the time of service delivery).  Collaborates with support staff to facilitate efficient scheduling of appointments and billing for services provided, including keeping the Main Office Calendar up-to-date.   Provides on-call emergency services on a rotational basis.    Must be a licensed or certified mental health professional (as defined in State Standards).   Skills/Qualifications: State certification in addictions counseling or LMFT, LPC. LCSW with min 2 yrs experience in addictions
