Supervise H4H staff Evaluate prospective resident referrals, determining preliminary eligibility. Conduct intake, recertification and exit interviews. Ensure H4H staff participation as a member of case management teams to develop client goals and monitor client progress. Conduct team interventions to support Case Managers when residents are in serious non-compliance with their service plans, engaging in behaviors destructive to their goals or facing formidable difficulties not readily addressed by Case Managers. Conduct program orientations for new Case Managers and Sponsoring Agencies. Conduct Sponsoring Agencies meetings to reinforce program guidelines and provide training. New Parent Support Home Visitors will support expectant and new parents at US Army installations in Germany, Belgium and Italy. Services provided will augment and expand existing Family Advocacy Program services through outreach efforts. Programs and services for expectant and new parents contribute to mission readiness, support family adaptation to military life and are designed to enhance the knowledge and skills families need in order to form health relationships and provide safe, nurturing environments for children. Specific duties include: - Visit families with children prenatal through three years of age in their homes to assess for risk of child maltreatment and/or family violence, to provide information and support with pregnancy and parenting education and to develop and build on family strengths. - Provide parents support and practical information and teach appropriate skills for parenting to enhance abilities to nurture and care for infants and small children. - Provide at least one initial contact/visit to new parents prior to birth and continue to work with parents on a voluntary basis during the critical first six months, and if necessary, the first three years of the child's life. (Provide services to eligible families with children above the age of three if all Army family requests for services to ages zero through three are being met.) - Review preliminary assessments with other home visitors or the installation Family Advocacy Program Manager (FAPM) to determine level of risk and to discuss/plan possible services. If the family chooses to participate in the home visitation program, the home visitor and family will develop goals for areas of parenting and family life the family wants to achieve, agree on the methods and a time frame to carry them out, sign appropriate informed consent and review the reporting requirement for incidents of child, intimate partner violence and sexual assault. - Refer reports of abuse to the community Report Point of Contact and installation FAPM. - Offer activities and services to the families that include but are not limited to educational courses, playgroups, and support groups. Participation by family members in these services will be voluntary. Services will emphasize a coordinated community response and every effort will be made to include both parents. The needs of the family will be addressed through a written Family Service Plan that is tailored for each family and reflects cultural, linguistic, racial and ethnic diversity of the American population served. Primary, secondary and tertiary prevention services are offered through this program. High needs families who accept home visitation services will receive at least two home visits each month. Relocation assistance is available although preference will be given to individuals currently living in Germany, Italy and Belgium assuming they are eligible for SOFA status.
