Job Title: Social Worker - Casual Company: Ministry Health Care Location: Marshfield, WI Description: The Department of Case Management and Social Services exists to assist patients and their families with the many complex clinical, social, emotional, financial, and continuing care needs that may arise from a hospitalization. Services are provided to meet these needs while a patient is hospitalized and also to help ensure a smooth and medically appropriate transition to the post acute care environment.The department provides services to all patients and their families, regardless of age, diagnosis, religious or cultural backgrounds, or financial status. Department members provide direct services, but also work collaboratively with other members of the health care team to help ensure comprehensive and holistic care. We are looking for someone who is willing to work 4-5 days per month, during the week. This would also involve every 6th weekend.
