Coordinates discharge planning by initiating discharge plan upon admission, reviews estimated length of stay, identifies discharge problems with treatment team during treatment team meetings and maintains contact with program therapist and physician to assist in aftercare planning. As necessary directly assists patients in obtaining resources, placement options and aftercare arrangement. Assist patient in making calls and inquiries when necessary.
