Children's Rehabilitative Services (CRS) is a managed health care program for individuals ages birth to twenty-one with special health care needs. The mission of Children's Rehabilitative Services (CRS) is to improve the quality of life for children by providing family-centered medical treatment, rehabilitation, and related support services to enrolled individuals who have certain medical, handicapping, or potentially handicapping conditions. POSITION SUMMARY: The CHS Social Worker II evaluates need for social work services, performs psychosocial evaluations and assessments, and organizes case management to Children's Rehabilitative Services (CRS) members. For CRS members who are transitioning to adult services, the CHS Social Worker II initiates and coordinates an ongoing transition plan until the member leaves the CRS program. Education Required: Master's degree in Social Work and licensed through the Arizona Board of Behavioral Health Examiners. Experience Required: Three years experience in pediatrics, community services, group process and crisis intervention. Experience Preferred: Bilingual (English/Spanish) ' Speak, Write & Read. YRMC will accept equivalent combination of education and experience

Under supervision, the incumbent will work full time ( M-F, 8 hours per day) at Ironwood State Prison and Chuckawalla Valley State Prison (Blythe, Riverside County) as an Institution Based Social Worker managing complex, multiple and full caseloads consisting of Inmates requiring long-term medical care, inpatient, mental health care, board & care/assisted living, in-home health care, and hospice; Inmates diagnosed with HIV/AIDS, chronic illness (i.e., dialysis, continuous oxygen); Mentally ill inmates designated Enhanced Outpatient Program (EOP), Correctional Clinical Case Management System (CCCMS); Inmates that are developmentally disabled and/or have other qualifying disabilities; Inmates that are 65 years of age or older; Inmates that will reside with and be the sole guardian of minors upon release, (Medi-Cal eligibility presumption). Independently conduct bio-psych-social assessments, complete Central File, and Unit Health Record reviews, develop and revise each client's service plan, determine potential benefit eligibility based on medical/mental disability and/or age, gather necessary information for the benefit applications, complete and submit benefit applications, provide case management post release as needed, identify and refer inmates that require community-based continuity of care upon release from incarceration. Act as a liaison with institutional and parole staff as needed for cases that may require additional case management, and collaborate on specific cases, as necessary. ~~Must be willing to work full time in a minimum thru maximum security state prison. QUALIFICATIONS: * Masters degree in Social Work or related field required and possession of the requirements necessary for licensure as determined by the California Board of Behavioral Science Examiner. * Knowledge of various psychological theories, therapeutic interventions, pharmacology, and differential diagnosis using DSM-IV and ICD-10. * Proven ability and experience to work effectively with other mental health/recovery professionals, correctional and parole officers on behalf of clients. * Proven ability and experience working with community mental health resources, and benefit providers on behalf of client. * Proven ability to consolidate clinical information from a variety of sources into assessment and the treatment planning process. * Demonstrated ability to think creatively, to assess, analyze, prioritize and solve an array of clinical and psychosocial problems. SPECIAL CONDITIONS OF EMPLOYMENT: ~Must possess a valid driver's license in California and a clean driving record. ~Annual TB test required. ~Must be eligible for clearance to visit and work inside the prisons.
