When appropriate andfeasible, will educate and encourage the veteran/family to advocate on their own behalf, thus fostering a sense of independence and empowerment. 6)Crisis Intervention -make rapid assessments and develop crisis management plans for admission to mental health in patient programs. 7)Education/Health Promotion and Prevention - provide education related to VA and community resources, entitlements, Advance Directives, and will refer veterans and their families to the appropriate interdisciplinary team member for identified health education needs. 8)Coordination of non-institutional and institutional services - reviews the progress notes from other providers to accurately determine the strengths and limitations of each veteranbeing referred for mental health or addiction treatment services. 9)Supportive counseling - provides the veterans and their care givers with ongoing supportive counseling to help deal with the psycho-social impact of coping with chronic/disabling illness(es) and/or the onset of catastrophic illness. 10)Administrative responsibilities - all veteran/family contacts will be entered into the electronic records; will attend all appropriate CBOC staff meetings; will collaborate in the performance improvement process and comply with performance measures; and will participate in mental health and social work peer review.Qualifications: Basic Requirements: US citizenship Master' s degree in social work from a school of social work accredited by the Council on Social Work Education. Licensed or certified at the Master' s level to independently practice social work in a state. Proficient in spoken and written English. NOTE: Persons, who were permanently employed as social workers, GS-0185, in VHA on or before August 14, 1991, do not have to satisfy the licensure and/or certification requirement. These employees may be promoted, demoted, or reassigned within the GS-0185 occupational series. However, if an employee initially retained in this manner leaves this job series, either voluntarily or through adverse action, the employee loses protected status. If subsequently hired or reassigned as a social worker in VHA, that employee would have to meet the full requirements (including licensure and/or certification) in effect at the time of reentry.
