We are an approved NHSC Student Loan Reimbursement Site. We are currently looking to hire an In-Home Counselor (PLPC or LPC). Qualifications include a Master`s Degree in the counseling field, ability to acquire LPC or PLPC credentials/license in the State of Missouri and 1 year of experience working with children and families. Responsible for providing service where youth and family live and function (in the client?s home) ? Work to defuse any crises, evaluate their nature, and intervene to reduce the likelihood of a recurrence ? Coordinate with team members to act as a single organizational unit while providing services ? Provide services that are accessible and flexible to families ? Work in tandem with team members to coordinate services and follow-up assessments. ? Work in tandem with supervisor to facilitate the referral and discharge process, producing or obtaining all necessary paperwork, for any and all intensive in-home services needed by assigned consumer. ? Responsible for the monitoring of recipients? Individual Treatment Plan ? Conduct and provide updates on recipients? Child and Family Treatment team meetings. ? Ensure linkage to needed community services and resources. ? Provide self-help and living skills training for youth. ? Provide parenting skills training to help the family build skills for coping with the youth?s disorder. ? Monitor and manage the presenting psychiatric and/or addiction symptoms. ? Work with caregivers in the implementation of home-based behavioral supports. Services may include crisis management, individual and/or family therapy, substance abuse intervention, skills training, and other rehabilitative supports to prevent the need for out-of-home, more restrictive services. ? Advocate on behalf of the family with other providers ? Document all contacts with recipient in Medicaid approved format. ? Responsible for submitting Medicaid documentation by specified deadline ? Attend weekly team case consultations This is a contract position.
