Actively identifies obstacles to achieving positive outcome, and works with the treatment teams, supervisors and managers to resolve the obstacles. Assumes responsibility for conducting all Utilization Review for the patients and works with payers in determining covered days. Coordinates discharge planning with patients, families and outside agencies as necessary, through individual and family meetings, telephone contacts and interagency conferences, to provide our patients’ insurance providers with information pertaining to hospital level of care. The discharge planning interactions are documented in progress notes. Reviews/discusses discharge planning activities at team meetings, and documents these twice weekly at treatment plan updates. Attends multidisciplinary team meetings, and provides input to team members regarding psychosocial problems, overall treatment objectives, and treatment and discharge planning for each patient as reflected in written documentation. • Staff Education  Participates in educational and other activities that helps maintain clinical competency in areas of responsibility to ensure professional credentials. Attends and participates in staff meetings. Attends and participates in all mandatory in-services. Attends and participates in voluntary in-services.
