General Statement of Responsibilities: The Family Support Coordinator is responsible for providing high quality home-based case management services to families identified by the Department of Mental Retardation. Services include, but are not limited to: assessment, development and implementation of service plans, referrals to community based agencies, crisis intervention and guidance in the use of a state financial allocation. The Coordinator develops with the family a yearly Family Support Plan indicating anticipated support. The Family Support Coordinator is directly responsible to the Developmental Disabilities Director. Hours: 37.5 hours per week; evening hours maybe required for home visits, meetings or community activities. Specific Responsibilities: 1. Maintain caseload as assigned by Director. 2. Conduct at least two home visits per year, one of which is to assess and involve family in development of a Family Spending Plan. 3. Assist families in accessing goods and service that enable them and the family members with a disability to improve their quality of life. 4. Maintain contact with all assigned families as outline in the service plan 5. Maintain record for each family on the caseload. 6. Recruit, interview and hire respite workers and skill trainers for individual families. Supervise and support respite workers and skill trainers assigned to families on caseload. Identify, coordinate and provide respite worker and skill trainers professional development. 7. Maintain current information on resources including vendors, prices, contact persons etc. 8. Participate in the development and coordination of family activities and other program components as assigned by Program Director. 9. Participate in advocacy efforts to make the family support programs more accessible to a multicultural population 10. Participate in the annual evaluation family satisfaction 11. Attend agency and program staff meetings. Interface with other program staff to assure communication, consistency and teamwork among staff. 12. Set and observe appropriate boundaries with clients; observe client confidentiality and HIPAA protocols. 13. Participate in ongoing supervision 14. Participate in professional development 15. Maintain accurate weekly schedule 16. Maintain appropriate documentation of case management services; Maintain thorough and accurate records, files, correspondence and statistics in both written form and via data entry in a computer database. 17. Maintain and promote the cooperative, harmonious, teamwork environment CFCS strives to foster in the workplace; maintain and promote an atmosphere of dignity and respect in line with the philosophy and policies of CFCS. 18. Other duties as assigned Qualifications, Skills & Abilities: 1. Bachelor?s Degree in Social Work, Psychology or other related field. 2. Bilingual preferred. 3. Minimum one year experience working with developmentally disabled population. 4. Demonstrated knowledge and experience with issues facing program participants (e.g. substance use, mental health, domestic violence, resources for undocumented individuals, poverty, etc.). 5. Experience working with mental health related issues, substance abuse, domestic violence, HIV/AIDS related issues, Child Protective Service agencies, HIPAA and crisis intervention techniques; 6. Able and willing to work with diverse staff and client populations. 7. Excellent written and verbal communication skills; computer literate ? ability to operate IBM compatible computers including regular entries into a web-based client database and demonstrated ability to use MS programs such as Word, Excel, Outlook, PowerPoint, Access. 8. Must be able to perform extensive charting, data entry and documentation. 9. Background check required (pre-offer). 10. Possess or be able to obtain CPR and First Aid certification. 11. Valid drivers license and clean DMV record strongly preferred; Able and willing to travel locally as needed.
