JOB SUMMARY: Responsible professional position involving assessment, screening, networking with referral sources and on-call responsibilities for the Residential and Outpatient Mental Health Services. ESSENTIAL FUNCTIONS: 1. Coordinates admissions for the Residential and Outpatient Mental Health Programs. 2. Implements intake policy and procedure admissions, including monitoring compliance with paperwork requirements and meeting the agency census goals. 3. Screens and assesses children and families for appropriateness in the programs and makes all necessary referrals if other services are needed. 4. Completes registration process in the Avenues system for clients and families. 5. Completes a written summary of child histories and recommendations within given time frames and to support timely Medicaid billing. 6. Conducts face to face interviews with children and parents. 7. Works closely with HKI placement office and CHI residential staff to schedule and implement emergency and respite residential admissions. Requires on-call 8. Functions as contact/case manager for short-term/emergency admissions. 9. Develops, maintains and structures delivery of admission services, develop and participate in Continuous Quality Improvement programming and maintain the highest quality delivery of client services. 10. Networks with referral sources including Hillsborough Kids Inc, The Department of Children and Families, and Camelot Inc., and referral sources from other counties, presenting in a professional manner which creates a positive image of our agency. 11. Seeks feedback from Clinical Services Director, Clinical Services Supervisor and Residential Management Staff regarding admissions. 12. Provides training for CHI staff in areas of expertise. 13. Assists in the peer review process for Residential and Outpatient Mental Health files. 14. Works within the philosophy, function and Personnel Practices of The Children's Home Inc. 15. GENERAL DESCRIPTION: Primary responsibilities: To manage daily operation of programmatic area of Child Care Resource and Referral and eligibility in regards to parental services for Putnam County. EDUCATION AND EXPERIENCE: ? Bachelor?s degree in Early Childhood Education, Business Administration, Nonprofit Administration or Accounting. ? 3-5 years related work experience preferred. MAJOR RESPONSIBITIES: 1. To provide child care information to all parents regardless of income level. 2. Ensure the facilitation of parental choice through the identification of available services, and the provision of information and assistance, which helps parents to evaluate the appropriateness of different child care settings and make a choice of what is best for their needs. 3. Ensure that child care choices are offered free of bias and preference for any specific type of child care; center based, home setting, faith based, public or private. 4. Be able to communicate knowledge in the area of developmental needs of children, local and state care rules and regulations and developmentally appropriate child care practices. 5. Convey child care information in a relaxed manner, problem solves in a positive manner and communicates sensitively and responsively to the needs and concerns of others. 6. Manage bi-annual provider updates and ensure accurate information is entered into EFS. 19. Review DCF Master Facilities and Homes list to monthly to ensure all providers on DCF list are in EFS. 20. Complete monthly snapshot report and other reports as needed for ECS, ELC of PSJ, and/or AWI/OEL. 21. Assist in other areas of CCRR as necessary.
