Normally from 9:00 AM until 6:00 PM Job Summary: The Case Manager is responsible for the screening, assessment, treatment planning, and case management for assigned participants in an Independent Living Program. Essential/ Function Responsibilities: Obtain, review and interpret all relevant screening, assessment, and treatment planning information to determine the participants' strengths and areas of need. Formulate individualized service plan with specific, measurable, behavioral objectives and action steps; and document treatment process, progress and outcome on a regular and consistent basis Facilitate the participants' engagement in their independent living skills program. Maintain participants records that are accurate, comprehensive and, in which the documentation is completed on a timely basis. This includes clinical and case management services. Conduct individual, group and family counseling to assist participants in their insight development and individuals services plan goals achievement. Understand and apply state and federal confidentiality regulations with regard to participants' records and correspondence. Contribute actively as part of a multi-disciplinary treatment team. Prepare Individual Service Plan (ISP), reports and legal correspondence and submit to referral agency like Child and Family Services Administration (CFSA). Maintain ongoing contact with CFSA social worker to update them to the clients' progress in treatment. Requirements: Bachelor's degree in the human services field (counseling, psychology, social work) or five years of experience providing treatment and case management services to young men and women with Serious Emotional Problems. Excellent oral and written communication skills; working knowledge of Microsoft Office, knowledge of counseling principles and techniques; comprehensive knowledge of clinical components of treatment and their application in working with adolescent population; and a valid driver' s license.
