Job Title: Social Services Director Company: SunBridge Healthcare Location: Stamford, CT Description: Celebrate Caring at St. Camillus Care and Rehabilitation Center, an affiliated center of SunBridge Healthcare and a leader in long term and rehabilitative healthcare. We are currently seeking a full time Director of Social Services at our 124-bed center located in Stamford, CT.   As the Social Services Director you will be responsible for performing assessment of residents upon admission, change of condition and review and update care plans; provide direct psychosocial intervention; assist with family in coping with placement, illness and grieving; coordinate outside services and educate staff.     * A Bachelor's of Social Work or related field degree required. * Previous experience in long term care environment or skilled nursing facility. * Good working knowledge of psychosocial practices that apply to a long-term care/healthcare environment. * Ability to direct and motivate staff.
