Ensure program success by creating and monitoring weekly goals and objectives that lead to self-reliance moving client families to permanent housing within 24 months. Maintain adequate records and reporting documents for caseload. Work closely with project director, property manager, and case manager. MAJOR DUTIES & RESPONSIBILITIES: • Conduct preliminary screening for potential clients. • Schedule, attend, and perform intakes with potential participants eligible for the Safe Places Program. • Provide direct services to a case load of ten (10) families participating in the Safe Places Residential Program • Conduct weekly scheduled home visits/case management sessions at scattered-site housing units located throughout Fairfax County. • Provide case management, guidance and support focused on trauma recovery to resident families for impacts related to domestic violence and other adjustment difficulties or personal barriers. • Assess, model, monitor and refer life skills such as independent living, parenting, budgeting and home care, etc. • Assist clients in obtaining needed services such as employment, childcare, health care, substance abuse treatment, mental health treatment and transportation, etc. • Advocate with various systems, including but not limited to the police, schools, and courts. • Complete monthly service updates, evaluation tools and other required reporting documents. Write weekly process notes and case summaries. • Co-facilitate Monday Night children’s educational support groups. • Recruit and screen volunteers for Monday Night Educational Support Groups and other special projects. • Other assistance as needed to move families from homelessness towards self-reliance within 24 months.
