Duties include: ? Provide case management services to clients in their home (residence), at the Center, in the community and other institutions, (such as ACI, hospitals, substance abuse treatment settings.) ? Assess and evaluate all clients on an ongoing basis, providing support services and/or crisis intervention as needed ? Develop, design and implement treatment goals/plan with client Spanish speaking abilities and knowledge of substance abuse a plus. Ability to work across multidisciplinary teams, driver's license, approved driver's record, and reliable transportation required. One year experience in community mental health strongly preferred. Education Requirements: AA in a human services field required; BA/BSW in a human services related field preferred. License/Certification Requirements: Valid driver's license.
