We are seeking a Senior Supervising Case Manager to lead a team of case managers in delivering services to homeless women at HomeFront's Family Preservation Center. The Senior Supervising Case Manager will provide case management services and implement strategies consistent with the philosophy of HomeFront with the specific objective of assisting families to secure and maintain permanent housing, a stable condition and self-sufficiency. Responsibilities: * Provide individualized case management services for clients. * Conduct potential client intakes, interviews and orientation. * Conduct, monitor and update in-depth needs and strengths assessment of clients. * Develop, monitor and update Action Plans for each client. * Participate in placement of clients in sites appropriate to client needs and preferences. * Supervise and mentor staff. * Develop and maintain resource lists of programs and services. * Develop and maintain cooperative working relationships with other social service agencies and volunteers. * Review service plans and encourage client participation in plan. * Compile and report monthly statistics regarding clients. * Maintain a case management tracking system that is responsive to changes in client needs and service providers. * Insure and protect agency, employee and client confidentiality. * Other related duties as necessary Qualifications: * Bachelor's Degree in Social Work, Sociology, Psychology or related field. * Five to ten years of demonstrated experience working with low income clients and/or homeless families in a social services setting. * Demonstrated experience with addiction, mental health and family issues. * Must have prior hands-on experience supervising and mentoring staff. * Expertise in the fields of substance abuse, mental health, employment, education and/or housing. * Excellent verbal/written communication skills. * Working knowledge of Microsoft Office applications. * Proven attention to detail. * Demonstrated organizational and problem solving skills. * Ability to thrive in fast paced work environment.
