TYPICAL RESPONSIBILITIES AND DUTIES:Although it is not possible to provide an exhaustive list of all types of duties, responsibilities and performance standards, the following are the essential job functions:1. Prepare written personalized program for each resident to establish measurable criteria of expected behavior and accomplishment and a time frame for achieving specified goals. Review plan with resident on a bi-weekly basis.2. Assist resident in establishing a personal budget, locate suitable housing, and participation in family and community activities, as appropriate, to ensure they are working towards financial independence.3. SKILLS / ABILITIES:Able conduct individual counseling sessions and facilitate small groups; interface with outside agencies. Write clear reports in English, using correct grammar. Able to add, subtract, multiply, divide, and calculate percentages. Ability to effectively interface with residents and staff supervise residents and react quickly and appropriately in an emergency situation.JOB REQUIREMENTS: 1. Personal vehicle available for company use. 2. Valid driver’s license and insurance for state in which facility is located. 3. The Lead Case Manager will also work in conjunction with nursing, physicians, ancillary hospital departments, patients and families to identify and coordinate post hospital needs, resources and services to all patients, infant to geriatric in order to promote quality, continuity of care and safety for the patient. He/she is responsible for carrying out physician orders regarding transfers, home health care, durable medical equipment and the specific discharge needs of the patient, placing patients in the most appropriate setting upon discharge. He/she will monitor the utilization of resources, track any avoidable days identified, respond to insurance requests for clinical reviews in a timely manner utilizing Interqual criteria and follow review protocols for the department . Qualifications Education: Graduate of a nursing program - diploma, associate degree or baccalaureate program (BSN Preferred) will consider BSW or MSW from a school or university. License: Current Nevada R.N. License or Current Nevada Social Worker License Certification:             Current Basic Cardiac Life Support required Case Management certification preferred Experience: Minimum of 2 years case management experience required, preferably in an acute/subacute facility. Special Skills: Must have strong communications skills, Knowledge of PC and, Interqual criteria. Prefer knowledge of local community resources
