Case Manager CLASSIFICATION: Full Time / Exempt REPORTS TO: Program Manager GENERAL SUMMARY: Under the general supervision of the Executive Director and the Program Manager, the Case Manager assists with all aspects of the transitional housing program consisting of 15 members and up to 25 children and the continuum of care component of the program. Responsibilities include connecting the essential housing service components of the entire program: prevention/shelter/tran sitional living/skill development/follow-up and other administrative duties. The Case Manager ensures full implementation of and compliance with the program. The Case Manager is available and on-call for the program and assists with the delivery of services to the members. QUALIFICATIONS: -Bachelor's Degree in Social Work, Psychology, Education or appropriate field as required. If hired without a Bachelor's Degree, candidate must be working towards acquiring their degree. If during a two to three year period it is apparent that candidate cannot complete the degree, candidate may be reassigned to a position they qualify for or may be terminated. -At least two years experience working with women and children. -Valid driver's license and a good driving record. -Chauffer's license. -Reliable transportation. PRINCIPAL DUTIES AND RESPONSIBILITIES: -Directly responsible for all aspects of the program under supervision of the Program Manager and Executive Director. -Manages and executes the continuum of care component with assistance and guidance from Program Manager. -Assists on the completion of detailed case plans for members and monitors the implementation of such. -Participates in the decision making of termination of members from the program and follows up with regard to the after- care prospects. -Maintains member and staff confidentiality. -Participates in weekly House Meetings for the members with the Program Manager. -Manages crisis situations with members and staff in an efficient and professional manner. -Monitors the health of all members by keeping up with appointments, wellness checks, immunizations, etc. -Assists in enrolling members in school, scheduling appointments and all other case management tasks under the continuum of care program. -Works closely with members and the Accountant in completing all ?finances? for the members while they are in the program and upon exit from the program. -Maintains the members' status with address, receiving of welfare, benefits, food stamps, daycare assistance; school and health related enrollments for members. -Arranges for members to procure child care -Work closely with therapists to ensure that member needs are being met. -Works with Program Manager on completing all program related Documentation: ˘Makes recommendations for the Member Handbook ˘Reviews, signs and makes recommendation on all Incident Reports ˘Prepares disciplinary documentation for members in consultation with Program Manager. ˘Provides information for all tracking services (ROSIE, internal client database).
