Job Title: SPMI/SED Case Manager - (Somali Language) Company: University of Minnesota Twin Cities Location: Minneapolis, MN Description: Duties/Responsibilities: Essential Functions & Percentage For Each Function: 1. Provide adult case management services These include, per the Dept. of Human Services and State of MN Statute: - Develop and review recipient's Individual Community Support Plan. - Monitor the discharge planning for a recipient who is leaving a residential facility, regional treatment facility or inpatient hospital. - Assisting a recipient in obtaining health coverage for covered medical services. - Arranging for transportation, lodging, meals, or child care needed for recipient to access service. - Assessing and evaluating the effectiveness of services received by the recipient. - All planning, assessing, record keeping and documenting associated with the service coordination of a case. - Receiving clinical supervision for an identified recipient with SPMI or SED. - Routine contacting and other monitoring or communicating with the recipient, family members, or other relevant persons about the status of the recipient, the individual service plan, or progress in achieving the goals of the service plan. - Developing goals, service plans, written service agreements and routine case supervisory activities. - Completing and regularly reviewing the written functional assessment. - Assisting a recipient in obtaining needed services. - Coordinating with other agencies to obtain services for recipients. 2. Coordinate psychiatry services for recipients Arrange for the recipient to access psychiatry care as outlined in their service plan, including assistance with transportation. May act as interpreter for psychiatry or other mental health outpatient clinic service visits. Required/Preferred Qualifications: REQUIRED QUALIFICATIONS (MUST be mentioned on application/resume): 1.BA/BS in one of the behavioral sciences or related fields including but not limited to social work, psychology, or nursing from an accredited college or university OR without a degree must have 4 years experience as a case manager. 2. Fluency in written and spoken English and Somali. Preferred Qualifications: 1. Access to transportation to make home and agency visits. 2. Experience working with clients with mental illness. 3.
