Pine Street Inn Case Managers for the Home to Stay (Housing Occupancy and Maintaining Employment) program will provide rapid housing, continuous supportive services and Work First opportunities to chronically homeless adults. Case management will provide a program structure for supporting chronically homeless individuals transitioning into permanent housing in scattered site and congregate settings according to the program?s established goals. Case managers from PSI, LI, Woodsmullen & Hopefound will work together to prioritize referrals and conduct screenings for placement in the Home to Stay project. The case manager is the primary point of contact for each assigned client and is responsible for working with the client to develop an individual service plan designed to support the client?s long-term stability in housing. Clients will be connected by the case manager to a housing advocate at HomeStart and to Hopefound/Work First for employment services. The case manager also links project participants with mental health and substance abuse treatment; treatment for trauma and co-occurring disorders; and conducts follow-up activities.
