ESSENTIAL JOB FUNCTIONS:* ·        Assists clients in developing the knowledge and skills and acquiring the necessary supports to live as independently as possible in the least restrictive setting in the community, including budgeting/money management, hygiene, housekeeping, nutrition, shopping, representative payeeships as needed and community integration activities.   ·        Helps clients identify strengths, set goals and develop a life plan; participates with team members in the development of a holistic assessment of the client, including physical, emotional, psychological, and spiritual.   ·        Participates with client and other staff in the development and implementation of a plan that supports and advocates for the attainment of client's life goals.   ·        Provides supportive counseling and psycho-educational services to individuals and others as needed, both on and off site.   ·        Provides crisis intervention, assessment, referral, and follow-up in an environment safe both for the client and staff.   ·        Collaborates with family members and significant others as appropriate.   ·        Provides family education and consultation, if desired, by a person receiving community support services in order to help family members and/or other natural supports to develop support systems and help the person to manage their mental illness; maintains proper documentation in the client's record.   ·        Ensures client confidentiality is maintained at all times and in accordance with federal, state, and agency standards. CIS Case Manager Page 2 of 4 ESSENTIAL JOB FUNCTIONS (continued):* ·        Meets established direct service expectations; maintains client records in accordance with agency and licensing standards and Medicare and Medicaid regulations.   ·        Participates in scheduled meetings, including case reviews, supervisory sessions, staff meetings and other inter-agency and intra-agency meetings as appropriate for program and clients.   ·        Completes and submits accurate, neat, and legible records, logs, forms, and reports in a timely manner and according to policies and procedures. OTHER DUTIES AND RESPONSIBILITIES: ·        Participates in the provision of services in a team approach, networking with other providers and providing back up services in the absence of other staff.   ·        Represents the agency to the community in a professional manner by demonstrating a professional attitude, sound judgment, and continuity in interactions with external customers such as the legal system, the medical community, and the Department of Human Services.   ·        Maintains and utilizes knowledge of community resources.   ·        Provides public education and professional consultation services, when appropriate.   ·        Adheres to agency, unit, and program policies and procedures including agency Risk and Safety Management Programs.   ·         Uses personal vehicle as may be required for client transportation, outreach in the community, home visits, and/or agency related business.   ·        Adheres to organizational standards for performance (see attached).   ·        Performs other related duties as required. ADDENDUM (ACT TEAM): Provides Case Management Services including: Street and community outreach Housing location/stabilization Insures the provision and maintenance of basic needs (food, clothing, etc.)   CIS Case Manager Page 3 of 4 ADDENDUM (ACT TEAM) Continued: ·        Links to and support with other services (vocational, educational, social/recreational, transportation, etc.)   ·        Links to and advocates for entitlements such as Mainecare   ·        Provides interface between ACT Team and other services providers around the services to specific clients   ·        Assists clients in obtaining non-psychiatric medical care   Provides on-call coverage and works shift assignments which will include some after hours, weekends, and holidays   Provides education with clients around symptom management, substance abuse, etc.            SKILLS/EXPERIENCE/TRAINING REQUIRED: ·        Minimum of MHRT-Community Provisional Certification required.  Duties also require knowledge of parameters and needs of adults with severe and prolonged mental illness equivalent to completion of a Bachelor's degree in a human services and/or equivalent combination of education and experience.   ·        Must possess a valid State of Maine driver's license.   ·        Ability to work in an interdisciplinary/interagency setting.   ·        Ability to negotiate, advocate broker and monitor services.   ·        Basic clinical and functional assessment skills.   ·        Ability to communicate effectively both verbally and in writing; to establish positive public relations; and to interact effectively with a wide variety of people.   ·        Ability to locate, use, and when necessary, develop resources for clients.   ·        Ability to work with community referral sources and caretakers.   ·        Good crisis assessment and intervention skills.   ·        Ability to engage persons with mental illness in developing rehabilitation plans and programming.   ·        Ability to be detail oriented and handle multiple tasks and priorities.
