Job Title: Supportive Housing Case Manager - Residential Counselor Company: Wyandot Center for Community Behavioral Healthcare, Inc. Location: Kansas City, KS Description: JOB TITLE:               Supportive Housing Case Manager/Residential Counselor ASSIGNMENT:         Community Support Services (CSS) FUNCTIONS:             To provide goal directed intensive services to adults with severe and persistent mental illness (SPMI) for the purpose of increasing functional skills, decreasing the need for hospitalization, and maximizing independent functioning in the community. Services will be provided in the consumer’s home and in the community. In order to assist the consumer in maximizing independent living, services will be provided day, evening and weekends. The team will operate under a continuous treatment model.    JOB LOCATION(S): ·         This position will be based at the CSS office location ·         To accommodate the needs of the program, work at various community sites will be necessary.   ESSENTIAL JOB FUNCTIONS: •       Conduct active outreach of consumers within the community. •       Establish a trusting relationship with the consumer, convey hope and respect •       Use motivational interviewing techniques to actively engage the consumer in services •       Identifies strengths and needs of consumers and works to involve them in recovery oriented planning for desired services •       Works to fully integrate consumers into the community •       Identify and address barriers to recovery •       Works closely with families of consumers to provide education and support. •       Link the consumer to other agency resources (peer support, community integrations, vocational services, outpatient services). •       Link the consumer with community resources (educational, cultural, recreational and social activities). •       Works closely with SRS, SSA, and other agencies as appropriate to secure financial support. •       Assists consumers in accessing desired housing and promotes stability through on going support. •       Provide on-site support to 4 consumers living in Beacon Homes supportive housing program, including helping to facilitate of sense of “community” among residents •       Conduct interviews of potential candidates and ensure eligibility of Beacon Homes residents •       Maintain a personal presence at Beacon homes that reinforces health, safety, and resident participation in a quality supportive housing program. •       Ensure residents at Beacon Homes maintain a clean, safe home through a process of inspection, encouragement, and life-skill training. •       Develop and facilitate a site advocacy council at Beacon Homes. •       Assist with data collection and reporting requirements related to residents at Beacon Homes. •       Provides crisis prevention and intervention as needed MINIMUM QUALIFICATIONS: ·         Possesses bachelor’s degree in social service field or related professional experience. ·         Ability to recognize and be sensitive to cultural and ethnic differences. ·         Ability to communicate directly and effectively in a cooperative manner. ·         Ability to relate positively with colleagues and representatives of other agencies. ·         Familiar with mental health theory and practice. ·         Must possess appropriate written communication skills. ·         Must be able to safely perform the essential job functions, with or without reasonable accommodations.    Please send resume and letter of interest to Register to View or fax to Register to View or mail to 757 Armstrong Ave., KCKS  66101 EOE

Job Title: RN (Registered Nurse) Case Manager Company: Preferred Health Systems Location: Wichita, KS Description: Job Requirements: Bachelors degree in health education, health care administration, public health, nursing, or social work. Current State of Kansas RN License required.Managed care related experience preferred. Two years experience in community health or primary care setting required. Facilitates the delivery of medical and/or behavioral health services provided to the member with providers, practitioners, other departments, and the Medical Director. Communicates total member /caregiver needs on high-risk cases by reporting changes to management. Responsible for maintaining a positive working relationship with member/caregiver, physicians, Home Health, DME providers, Inpatient Outpatient facilities and Ancillary Services, etc. Maintains open channels of communication with all health care team members.
