Job Title: Mental Health Intensive Case Manager Company: CONFIDENTIAL Location: Seaford, DE Description: Rapid Hire - Immediate Opening for Individual Contractor Mental Health Intensive Case Manager/Wraparound Facilitator for Sussex CountyDelaware's Division of Child Mental Health Services is seeking to immediately contract with one individual to provide professional services in Sussex County Delaware as a Mental Health Intensive Case Manager/Wraparound Facilitator on a multi-year federal grant project for comprehensive community mental health services for young children, ages 0 - 5 years, and their families. The contractor will have responsibilities in working directly with children and their families to assess for treatment services need, develop individualized treatment plans, work with family to identify appropriate service providers, monitor clinical treatment progress, identify/help obtain resources for non-mental health services to support the treatment plan, facilitate transitions and discharge from service. Intensive Case Manager/Wraparound Facilitator: Bachelors degree or higher in psychology, social work or a related human services field. Must have case management experience working directly with children and their families who receive mental health treatment.
