At least 3-5 years exp counseling/coaching and/or case management and be aware of psychosocial issues surrounding a person who has been recently incarcerated and released. 3. Since 1967, more than 100,000 drug-troubled individuals have received treatment at our adult residential centers, our adolescent Phoenix Academies, outpatient and day programs, and programs in prisons, drug courts, and homeless shelters. www.phoenixhouse.org Phoenix House is seeking a Case Manager who is responsible for the following: Collaborates with members of the multi-disciplinary treatment team for the delivery of a full spectrum of clinical services. Assists clients in meeting individual treatment plans, goals and objectives for assigned cases. Provides individual counseling services. Utilizes appropriate crisis intervention and de-escalation techniques in response to problems and/or behaviors which place the child or other residents in danger. Communicates with parents and other approved family members. Encourages and facilitates a family involvement in parent education, weekly visits, family recreation days, and other Phoenix House events. Interfaces with referral agencies such as Juvenile Probation, Department of Children and Family Services, and Department of Mental Health. Completes and submits Needs and Service Plans, Quarterly Reports, and Discharge Plans on a timely basis. Communicates with the Nursing Department and consulting physicians to ensure quality medical services are provided to the client on a timely and as needed basis. Completes and maintains all documentation of services delivered which includes individual records, group notes and general communication logs. The Case Manager is expected to perform their duties in alignment with the vision and shared values of Phoenix House. Qualifications: Two years (paid or intern) experience providing counseling or related services to adolescents. *Application Instructions: 1.Must possess a valid California Drivers license/ may be required to secure a Class B license 2.Excellent computer skills, writing and verbal communication skills 2.Non-Exempt from FLSA. 3.Incumbents in these classes will be required to transport residents to and from appointments. 4. Incumbents will be required to represent the treatment team recommendations in court. 5. Applicants not in possession of substance abuse counselor certification must register within first 6 months of employment.
