Job Title: Outpatient Case Manager/Para-Professional Company: Rivendell Behavioral Health Services of Arkansas Location: Benton, AR Description: Assists in the development and implementation of special therapeutic programs during school breaks. Connects the family and client with resources and services needed to meet basic needs. Provides follow-up and consultation with referral sources and/or involved agencies to ensure continuity of care. Assists with physician and nurse scheduling, providing transportation when needed. Demonstrates knowledge of developmental tasks of children and adolescents. Demonstrates the ability to recognize signs and symptoms of anxiety and impending behavioral escalation. Ability to recognize and evaluate behavior, changing levels of depression, tendencies toward self-aggressive behavior, cognitive functioning deficits, learning deficits, and patient’s capacity for self-care. Provides therapeutic interventions within scope of training and certification as specified, under the direction of the physician. Identifies signs of abuse and neglect and follow necessary procedures to ensure client safety. Demonstrates a willingness to be available to provide client-oriented services, when needed regardless of hours and times. Achieve productivity expectations through the completion of required clinical hours. Communicates accurate information to the treatment team as observed by the supervisor Works cooperatively and positively with all treatment team members as observed by the supervisor. Is flexible and creative with job requirements and expectation as observed by supervisor. Assists other members of the treatment team in arranging appointments with clients and families as needed. Progress notes reflect an awareness of the patient’s pathology and problems, notes are goal directed and follow the prescribed format (P.I.E.) as evidenced by the notes in the medical record. Documentation is submitted in a timely manner according to Outreach guidelines as evidenced by submission to UR Specialist. Provides accurate and complete documentation as observed by the UR Specialist.   Requirements Bachelor’s degree (B.A.) from four-year college or university.  Computer skills necessary.  Must posses or complete Mental Health Para-Professional training.
