JOB SUMMARY
Nationally recognized nonprofit organization seeks an Intensive Case manager who will be responsible for providing Intensive Case Management service for tenants living in AWA supportive housing in the Tenderloin neighborhood of San Francisco. AWA will house 36 chronically homeless adults and 48 chronically homeless families. Services will target tenants who have complex medical issues, severe and persistent mental illness and/or substance abuse/use. The ICM’s will work with the onsite tenant services staff to provide wrap-around services to tenants identified as having these complex issues. The Intensive Case Managers will provide 30 hours a week of direct case management and will manage a case load of 20-24 tenants at any given time.

KNOWLEDGE, SKILLS, and ABILITIES

  • Possession of a Bachelor degree in a behavioral science (social work, psychology, clinical psychology); Masters degree preferred or six years experiences providing ICM services to tenants with complex behavioral health issues and/or tenants with co-occurring mental health and substance-related problems.
  • Demonstrated ability to provide effective clinical services within a harm-reduction/voluntary services context and in-depth knowledge of harm reduction practices.
  • Demonstrated ability to provide intensive case management services to homeless individuals and families, especially those struggling with substance (ab)use and mental health issues.
  • Proven ability to navigate San Francisco behavioral health care system and experience in advocating for clients’ access to services, and collaboration with other agencies.
  • Experience in engaging difficult clients, crisis intervention, conflict resolution, and meeting facilitation.
  • Commitment to CHP’s Mission Statement.
  • Strong written and verbal skills and the ability to communicate effectively with a diverse population of staff and tenants.
  • Experience in organizing groups, group facilitation and group development.
  • Strong administrative and computer skills (MS Word, Excel).
  • Possession of 5150 card desirable.
  • Ability to speak Spanish a plus.
