(Community Support Team) Provisional Therapist Reports To: Executive Director Hours-Full-Time/Non-Exempt Compensation $20.00-25.00 hourly (depending on experience and education) Service Definition-Job Description This job responsibility is associated to adults with mental health, developmental disabilities and substance needs. Community Support Team (CST) is composed of three staff members Team Leader/QP, QP, & Paraprofessional (PP). CST is a direct and indirect periodic service in which the team provides direct intervention and also arranges, coordinates, & monitors services on behalf of the recipient & is provided in a range of community settings such as home, homeless shelters, libraries, etc. CST also includes telephone time with the recipient and collateral contact with persons who assist the recipient in meeting their goals. Services are documented in the PCP and cover an array of services such as skill building, assistance and support, individual restorative interventions for development of interpersonal, community coping and independent living skills. CST provider must ensure provision of first-responder services for all of the consumers. This includes either face-to-face or telephonically 24/7/365, and have the capacity to respond face-to-face within 2 hours, as well as have access to the crisis plans for consumers. CST provider ensures the consumer receives information and linkage to services needed; assistance accessing benefits such as SSI or VA; and there is on-going collaboration with other community partners on behalf of the consumer. Review service notes that the CST worker is providing consultation, information, assistance, and referral and linking, etc. Evidence such as intervention for specific activities identified on the PCP for verification that the CST members have assisted the consumer in accessing benefits. CST provider ensures service documentation is completed per Medicaid guidelines. Reviewer reviews documentation containing: a service note, purpose of contract, describes the providers intervention, time spent, effectiveness of intervention, and signature and credentials of staff providing the service. Education ? Completion of a Bachelor's Degree/Masters Degree from an accredited college or university in Human Science, Social Work, or Psychology or any related field. ? One year of Experience and maintain evidence of continued licensure sufficient to meet the requirements of 10A NCAC 27G .0104 Duties ? Greater Outreach services provides mental health and substance abuse services to adults under contract with The Durham Center. ? As Diagnostic/Assessment Provisional Therapist shall be responsible for conducting intensive clinical and functional face to face evaluations of recipients mental health or substance abuse conditions. ? Provisional Therapist will prepare Diagnostic/Assessment reports with recommendations regarding whether the recipients meet target population criteria. ? Provisional Therapist will perform a diagnostic/assessment for each consumer as assigned by Greater Outreach services. The assessment shall be conducted in conjunction with the licensed physician/licensed psychologist/nurse practitioner/physician assistant assigned to the case. Contactor shall use the diagnostic/assessment instrument provided by Greater Outreach services. But otherwise shall use independent professional judgment in rendering said services. Results from the diagnostic/assessment shall include an interpretation of the assessment information, appropriate case formulation and recommendations for immediate service needs. Provisional Therapist shall provide recommendations for the Person Centered Plan. ? Clinical oversight of the Provisional Therapist will be provided by Greater Outreach services Clinical Supervisor, in conjunction with the licensed physician/licensed psychologist/nurse practitioner/physician assistant assigned to the case. Greater Outreach services Chief Executive Officer or designee will provide administrative oversight. Requirements ? Provisional Therapist shall provide evidence of continued licensure sufficient to meet the requirements of 10A NCAC 27G .0104 for Diagnostic/Assessment services. Provisional Therapist shall also provide documentation for the personnel record that meets Greater Outreach services and State standards. ? Provisional Therapist will obtain and provide evidence of continuing professional liability insurance. ? Design of the treatment protocols employed within the agency ?
