Assessing the support systems available and needed for pediatric patients and siblings and for children of adult patients.12. Providing holistic family-centered care across treatment settings.13. Consulting and collaborating with the interdisciplinary team.14. Providing care consistent with Standards of Practice for hospice programs.15. Implementing care to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patient and family.16.
