Job Summary:
Social Worker is responsible for direct social work case management services to residents/participants.
Essential Job Functions:
Conducts the initial intake and assesses need and eligibility of potential clients meeting with family members and others. Obtains extensive social history from participant and/or family member upon admission to program. Maintains confidentiality of client information. Assists with ongoing financial eligibility for participants including recertification as needed. Initiates in home group home and nursing home assessments. Participates in participant-related conferences in the community as designated. Performs related duties as assigned.
Skills and Knowledge
Experience and thorough knowledge of social service principles and practices. Knowledge of psychosocial behavioral and family needs of the elderly population. Knowledge of financing mechanisms such as Medicare Medicaid Prospective Payment Systems. Knowledge of Sedgwick County health and social service delivery systems and aging network. Effective oral and written communication skills. Proven ability to work in an interdisciplinary team. Computer skills required.

Major Duties:
In this position, you will consult with commanders, managers, and supervisors to identify substance abuse issues and develop rehabilitation plans. You will review initial intake screening reports; collaborate with management officials to identify substance abuse issues; assess rehabilitation and referral needs; identify treatment modalities; review and modify treatment plans; establish treatment goals; and provide psychological counseling, treatment and consultation services. You will document case findings, diagnoses, and recommendations using specialized medical terminology, adhering to established formats and maintaining confidentiality of cases. You will plan the technical curriculum for educational awareness training; plan and develop complete training plans; conduct formal and informal training; assist in evaluating technical education and training classes; recommend needed class changes and improvements; and assess the adequacy of training products.

REQUIREMENTS TO QUALIFY: DEGREE: All applicants must possess a Masters Degree in Social Work at a school accredited by the Council on Social Work Education. The degree must have included a practicum and be at least 48 credits. LICENSURE: APPLICANTS MUST possess a minimum of Licensed Master of Social Work (LMSW) or be a Licensed Professional Counselor (LPC). EXPERIENCE: In addition to the above requirements, best qualified applicants will have one year of SPECIALIZED EXPERIENCE at the next lower level is required. Specialized experience is experience that has equipped the applicant with the particular knowledge, skills, and abilities to perform successfully the duties of the position, and that is typically in or related to experience in providing psychological treatment and consultation services to alcohol and drug dependent clients while in initial treatment, undergoing rehabilitation, and during follow-up care. STARTING SALARY WILL BE DETERMINED BASED ON YOUR LEVEL OF LICENSURE, CERTIFICATIONS AND EXPERIENCE. If you do not possess a Substance Abuse Certification, YOU MUST SIGN A CONDITION OF EMPLOYMENT STATEMENT TO ACKNOWLEDGE THAT YOU WILL OBTAIN SUCH CERTIFICATION AT YOUR EXPENSE WITHIN 6 MONTHS. If you do NOT possess a license that allows for independent clinical practice but you are a Licensed Professional Counselor (LPC) or a Social Worker with a Licensed Masters of Social Work (LMSW), AS A CONDITION OF EMPLOYMENT, you must obtain an independent practice license within a prescribed period of time. Upon obtainment of this license, YOU WILL BE NON-COMPETITIVELY REASSIGNED TO HIGHER LEVEL DUTIES THAT MAY RESULT IN AN INCREASE IN SALARY.
