We are seeking experienced MSW candidates dedicated to the patient/family focus of hospice and palliative healthcare delivery to service for our Blue Team service areas including: Blue Island, Oak Lawn, Burbank, Bridgeview, Oak Forest etc., and Red Team service areas including: Oswego, Sugar Grove, Aurora, Yorkville, and surrounding communities.
As a member of the hospice interdisciplinary team, the Medical Social Worker provides social service counseling and guidance to facilitate the maximum function and coping capacity of each patient and family member, while following all hospice policies and procedures, in accordance with the interdisciplinary plan of care. This position supports the Provider Relations activities of the organization, and assists the hospice physician and other members of the hospice team in understanding significant social, spiritual, and emotional factors related to the patient’s health problems in order to establish a plan of care that fosters the personal worth, spiritual well-being, and human dignity of each patient.
Required Qualifications:
Masters of Social Work from an accredited school
Licensed in the State of Illinois as an LSW or LCSW
Current CPR certification
Previous experience in hospice, home health or an acute care environment
Strong focus on customer service
Strong computer skills preferred
