The OEF/OIF Social Worker Case Manager works closely with the OEF/OIF Program Coordinator and RN Case Manager and reports to the Primary Care Clinical Manager. The incumbent's duties and responsibilities relate to the case management of severely-injured OEF/OIF service members and veterans treated at the facility. The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of OEF and/or OIF service members and/or veterans as they transition to VA care. The responsibilities include providing supportive services to families. In addition, the incumbent assists the service members and/or veterans in coping with acute illness, chronic illness, combat stress, the residuals of traumatic brain injury (TBI), community adjustment, addictions, and other health and mental health problems. The Social Worker Case Manager addresses home care needs, homelessness, and transition across the levels and sites of care. The case management responsibilities include psychosocial assessment, diagnosis, and treatment, is focused on helping OEF/OIF service members and veterans and their families to maximize rehabilitation and treatment potential and achieved more adequate, satisfying, and productive emotional and social functioning. Incumbent evaluates the need for mental health services and makes appropriate referrals for individual, group, marital and family treatment services. The incumbent uses the social work process in collaboration with interdisciplinary team members to develop a case management plan and psychosocial interventions and promote continuity for OEF/OIF service members and/or veterans.
