Job Title: FAMILY SUPPORT WORKER

Description:
With the goal of establishing a medical care home, provides outreach in the target community to patients who may be lost due to a lack of follow-up.  Participates in a variety of outreach activities to educate and inform the community of the availability and scope of primary and ancillary health care programs and services offered at Milwaukee Health Services, Inc.  Maintains a patient case load commensurate with the scope of daily job duties and responsibilities. DUTIES AND RESPONSIBILITIES             Assumes patient assignments and maintains a patient caseload in a professional and effective manner.             Performs home visits to locate patients lost to follow-up through referrals from medical providers.             Assists patients with making follow up medical appointments.             Becomes knowledgeable of the social services network in and about the City and County of Milwaukee.             Assists patients with appropriate referrals; internally to the Department’s Social Workers and other ancillary health care departments, and externally to referral specialists and other programs and services in the community.             Organizes and distributes an array of health education and health prevention materials, e.g. pamphlets, brochures, at a variety of community events (e.g., at health fairs, MHSI patient waiting areas/lobbies, in various MHSI departments) as appropriate.              Participates in data collection, record-keeping and data entry for his/her assigned caseload, and other Social Work Department activities on a monthly basis.             Participates in ongoing training programs for family support workers and attends additional continuing education activities as appropriate.             Attends departmental and team meetings and other meetings related to the Social Work Department and/or as required by MHSI Staff.             Advocates for patients within the community health center system and links patients with community based resources.   EDUCATION, TRAINING AND EXPERIENCE   At a minimum, high school diploma or equivalent.  College coursework and/or post-secondary school training or education is highly desired.  Experience working in a community based agency or community health organization preferred.  Valid State of Wisconsin driver’s license, ownership and daily use of an insured automobile.  Successful candidates need to be bondable.  Must participate in a mandatory training program for community health workers to be completed by June 2010.
