Our services include early childhood education (including Head Start and Early Head Start), mentoring and tutoring, emergency referral services, self-sufficiency/employment classes and services (including ESL and citizenship), job placement and training, substance abuse services, transportation and outreach for the elderly and disabled and publication of the Voice newspaper. Annually, Neighborhood House serves over 13,500 individuals. Of those, 80 percent are refugees and immigrants. Scope The Family Support Worker (FSW) has primary responsibility for assisting eligible individuals and families to become more stabilized and self-sufficient by providing comprehensive services. These services include outreach, crisis intervention, Family Development Model Case Management, linkages to other Neighborhood House (NH) and community-based services as well as technical assistance focused on building healthy, stable and vibrant communities. Age and needs of clients served may vary depending on funding, staff deployment or agency needs. Clients may include seniors, families of school-age children, low income families or individuals at risk of becoming homeless, for example. NH FSWs work in diverse communities where the majority of clients may have Limited English Proficiency. Bilingual FSWs are required to help immigrants and refugees navigate the US social support systems that can provide critical self sufficiency services. Interpretation and translation in individual and group settings are required. The position is located in the White Center area. Responsibilities ? Written and verbal translation in both English and a primary language (Vietnamese) that meets the cultural/language needs of NH clients and our surrounding communities. ? Maintain a caseload of at least 25-35 active clients using the Family Development Case Management Model. ? Provide intervention services focused on eviction prevention, family stabilization and changing at-risk behaviors. ? Provide a wide range of self-sufficiency advocacy, including asset building skill development, linkage to employment opportunities or access to adult/family education. ? Conduct outreach to residents and community members for NH services. ? Provide information, referral, and advocacy to link clients to appropriate services in a timely manner. ? Follow up with clients to ensure appropriate receipt of needed services. ? Limited accompaniment to services in the community. ? Maintain good working relationships with community service providers, government funders and housing authorities. ? Provide technical assistance to resident community building activities. ? Work as a member of the Neighborhood House Family and Social Services and interdepartmental teams. ? Participate in clinical case reviews, training, and case consultations. ? Maintain communication with team members, NH management and partners on client and community needs to help ensure NH continues to meet community needs and gaps. ? Maintain timely accurate, complete records of all clients in line with acceptable social work practices. ? Cooperate in the timely and accurate collection and reporting of service statistics using the NH MIS reporting requirements and forms. ? Assist team members with ongoing continuous quality improvement to enhance program operations, including problem solving, helping to develop program systems and procedures, or participating on team workgroups. ? Provide translation and interpretation services for designated language as needed. ? Work with staff from other Neighborhood House programs to coordinate services. ? Liaison and bridge to key ethnic communities and/or leaders to help NH coordinate services to underserved communities. ? Provide appropriate No Wrong Door Services to clients who do not meet other program eligibility, as needed. This may include co-case managing clients in other departments or providing translation or interpretation support for NH initiatives. ? Perform other duties as assigned.

DESCRIPTION: Provides coordination of support services for homeless families in emergency housing. Also screens, interviews, and places families in transitional housing and works with the family to develop self-sufficiency plan to establish and prioritize goals, identifying strengths and barriers, and develop an action plan to achieve goals. Provides coordination of support services for transitional housing families while they are residing in the transitional housing unit for up to twenty-four months. Provide follow up services for up to six months after the transitional housing family leaves the program. REPORTS TO: Program Manager LOCATION: South King County RESPONSIBILITIES Working with homeless families ? Assist with screening, intake and placement of emergency housing families ? Work with families to negotiate goals and objectives which encourage self-sufficiency and self-determination and which lead to stabilization in permanent housing. Work in partnership to create a family development plan utilizing a strengths perspective model. ? Screen families referred by service providers for placement when openings occur in transitional housing. Working with the program team, interview, screen and place appropriate families into the program. ? Meet regularly with families in emergency shelter, and transitional housing to build trust, coordinate service delivery, aid in overcoming barriers to goals, assist in negotiating the social service system and evaluate progress in meeting goals. ? Provide advocacy, information and referral in response to needs expressed by the family. ? Act as a liaison between mainstream service providers and families when needed. ? Work with the Program Manager to devise and implement a course of action plan for families not complying with program guidelines and/or policies. ? Assist in recruitment of prospective transitional housing participants, to include tracking applications received, emailing/faxing service providers announcement of openings, and responding to inquiries about announcement ? Provide follow up case management services with transitional housing families that have exited the program for up to six months. ? Provide conflict management ? Respond effectively to crisis situations Facilities ? Conduct regular walk through of all housing units ? Schedule cleaning of transitional and emergency housing units, to include reporting and monitoring repairs needed to the site property manager, as well as scheduling pest control services ? Ensure housing laundry is dropped off and picked up from Laundromat ? Stock emergency and transitional housing units with hygiene supplies, cleaning supplies and household items. Communicate with the Program Manager about needed items for stocking units and necessary furniture needs Community Relations ? Establish and maintain positive relationships with community service providers and advocacy groups. ? Participate in the planning, coordinating and implementation of educational projects that address issues expressed by families ? Participate in coordination and implementation of site enhancement projects; i.e. Day of Caring type projects. Respond to crisis situations Record keeping and Reporting ? Create and maintain family file for each family. The files will include all forms, paperwork and case notes as required by the contract of the program family is participating. ? Actively participate in regular supervisory and team or unit meetings ? Develop and maintain resource files ? Calculates, collects and receipts Program Service Fees from transitional housing families if appropriate ? Provides information to administrative staff for the completion of reports and database ? Completes transitional housing reports, to include Annual Progress Report and Annual Service Evaluation Other responsibilities may include ? Monitor incoming calls on the TTY/TDD for the SKC YWCA Regional Office ? Represent the YWCA in the community; including but not limited to committees and fundraising ? Ensure distribution of donations including YWCA adopt a family and school days ? Schedule appointments for Sharehouse for emergency and transitional housing participants, and other YWCA programs as needed ? Screen and intake participants for the Address Confidentiality Program through the Washington Secretary of State?s Office ? Other duties as assigned QUALIFICATIONS Either ? A Bachelor?s Degree with at least one year of direct social service experience working with low income and/or homeless families Or ? At least three years of direct social service experience, including one year working with low-income and/or homeless families. And ? Speaks Arabic or Somali fluently ?
