Job Title: Family Support Worker Company: Healthy Families Passaic County Program Location: Paterson, NJ Description: The Healthy Families Passaic County Program is seeking a Family Support Worker.  You will be responsible for initiating and maintaining regular and long-term contact with families enrolled in this program.  Responsibilities include developing/monitoring family services plans, conducting home visits, providing general health education information, maintaining files, and identifying/referring to supportive agencies.  Candidates must have the ability to interface with providers and clients in Passaic County. www.maternalchildhealth.org
