Job Title: Case Management Supervisor Company: Serco Location: District of Columbia, DC Description: Works with Navy Bureau of Medicine to help ensure Navy's case management program complies with the standards of practice of the Case Management Society of America. Provides Case Management Program Review services to BUMED on matters of coordination of care throughout the Navy. Serves as a subject matter expert on case management and related issues. Develops and implements tools to support case management such as those used for patient identification, clinical practice guidelines and databases for community resources. Assists with medically complex cases. Facilitates screening and assists with transfers of Exceptional Family Member Program families and coordinates as needed with other Navy programs. The goal is to provide a full range of services to the family to prevent out of home placement of one or more children. The Therapist will provide Family Preservation Reintegration services if children go into SRS custody; out of home placement. Job Title: Licensed MH Therapist Company: Community Connections Healthcare Services, LLC Location: Fayetteville, NC Description: Outpatient Therapy provides intensive clinical therapeutic services to children, adolescents, and adults with identified mental health or substance abuse diagnosis. This service is designed to meet the specific mental health needs of the identified client as they relate to their diagnosis and can be provided in-home, in-office, in-school or other approved/appropriate community setting.
