Works with Navy Bureau of Medicine to help ensure Navy's case management program complies with the standards of practice of the Case Management Society of America. Provides Case Management Program Review services to BUMED on matters of coordination of care throughout the Navy. Serves as a subject matter expert on case management and related issues. Develops and implements tools to support case management such as those used for patient identification, clinical practice guidelines and databases for community resources. Assists with medically complex cases. Facilitates screening and assists with transfers of Exceptional Family Member Program families and coordinates as needed with other Navy programs. Possess either 1) a current unrestricted license to practice as a registered nurse in any one of the 50 States, DC or Commonwealth of Puerto Rico, Guam or the US Virgin Islands OR 2) be a Licensed Clinical Social Worker in any one of the 50 States, DC or Commonwealth of Puerto Rico, Guam or the US Virgin Islands. The professional license must allow the holder to legally and independently practice without the supervision of another licensed professional.
