The Facilitator will assist in assembling a Child and Family Team to coordinate the development of the initial individualized Plan of Care and Crisis Plan. The Facilitator will monitor the condition and quality of services provided to the family, and provide supportive and crisis services to the youth and family. The Facilitator submits timely all required documentation and reports related to the Wraparound process.
