Description: AREA OF RESPONSIBILITY: The CSC is responsible for working cooperatively with the Program Manager and other staff in developing, implementing, and evaluating the outcomes of community living supports provided to individuals with developmental disabilities. The CSC is expected to contact the Program Manager regarding any program or personnel issues within the home as well as for emergency household, medical or maintenance needs in the home. The CSC is responsible for providing and coordinating direct services and healthful lifestyle supports to individuals with disabilities. These living supports include, but are not limited to: supervising, teaching and assisting the individuals in every aspect of their lives according to their Individualized Support Plans (ISP) with the goals of fostering increased independence, choice and empowerment; assisting maintaining a clean, safe living environment and assisting in meal preparation and completion of various household duties that are not included in the person's ISP and are not done in direct assistance to supporting the individuals with developmental disabilities. In addition, the CSC ensures the implementation of routine household activities, individuals' services and supports, and communication. QUALIFICATIONS: Minimum 18 years of age. High school diploma / GED required. Minimum of one (1) year experience required in the field of Developmental Disabilities or a Bachelor's degree in Human Services or related field. Valid driver's license in the state of Wisconsin, personal automobile liability coverage and a driving record that allows candidates to be covered by agency insurance policy required. Willingness to use personal vehicle to provide transportation for consumers depending on program needs (see program description). Also must possess all of the following: a sincere desire to provide individualized supports to persons with developmental disabilities; ability to make sound judgments when given guidance and priorities; ability to work as a team member to maintain consistency and quality supports; ability to communicate effectively with other staff members. DRIVING REQUIREMENT: Unlimited driving position -- defined as one that requires an employee to drive a motor vehicle as a part of his / her Dungarvin work, including vehicles owned or leased by Dungarvin, and / or to drive a vehicle in which a person served is transported.
