Long Term Support Counselor -

Location: Manchester, NH

Description:
In keeping with our mission, Easter Seals provides exceptional services to ensure that all people with disabilities or special needs and their families have equal opportunities to live, learn, work and play in their communities. We are currently seeking a Long Term Support Counselor to work with ServiceLink. Service Link provides community connections for elders and adults with disabilities and their families. In this role the individual will provide needs assessments, counseling and referrals, preliminary care planning and short term tracking for persons in need of Long Term Support. They will provide clinical eligibility counseling and financial prescreening for State and Federal Programs. Responsibilities ? Provide information and referrals by establishing contact with the consumer, assessing the individual?s long and short term needs, identifying resources to meet those needs, providing a referral to identified resources, and, where appropriate, following up to ensure that a consumer?s needs have been met. ? Assists with program operations as requested by the ServiceLink Resource Center Manager. ? Work in collaboration with BEAS staff to keep database of resources up to date. ? Assures all client records are maintained accurately by following Network standards for the use of Refer7. ? Appropriately refers consumers in need of long-term support counseling to the ServiceLink Resource Center Long Term Support (LTS) Counselor. ? Under the direction of the Resource Center Manager, assists with completing applications at consumer?s request if other services are not available. ? Assist in the coordination of preliminary care planning, long-term support counseling, and conducts research and provides limited follow up under the direction of the LTS Counselor. Qualifications: ? Bachelor?s Degree in Social Work, Human Services, Sociology, or Behavioral Science, Master?s Degree preferred. ? Strong communication skills, ability to multi-task and excellent organizational skills are a must. ? Three years experience in social work and case management; experience working with the elderly and or adults with disabilities is desired. ? Knowledge of state services and area resources a plus ? Must be flexible to work in either Manchester or Nashua NH.
