SOCIAL WORK ASSISTANTSince 1997, Extended Home Care has brought a range of quality care services to patients in the comfort of their home. Our continued growth in the NYC area has created a need for an Assistant Social Worker to work in Manhattan. You will be responsible for ensuring processes are followed in assigning patients to the appropriate social workers; and all administrative and clerical functions of the Social Work Department. Additionally, the incumbent will be responsible for Medicaid / Insurance Verification matters.PRIMARY DUITES:*Serve as contact person for the Social Work Department to address all social work concerns, needs, and problems.*Take directive from the Director of Social Work regarding coordination of all social work services.*Establish a reliable method of communication with each social worker.*Responsible for Medicaid recertification; NH conversion; non-covered; denial codes; as well as educating and assisting with pooled trust; spend down and bi-monthly mailings.*Cover for Medicaid Coordinator as needed.*Other duties and projects as assigned.We offer an attractive salary and benefits.
